If you would like to speed up registration on the inspection day, print out these forms, fill them out, but DO NOT SIGN THEM. Bring them with you and sign them in front of an inspection day volunteer. Please staple them in the proper order as listed below. Each person in your boat has to have their own stack.

The Float Plan should be on top. Put your WaterTribe name at the top. Print your name, date, Next Of Kin (NOK) with phone number. Your NOK is who we will call if you are dead or in the hospital. Only print out ONE of these per stack depending on your event.

Next is the WaterTribe Waiver which includes an Image/Audio Release. Read it and fill out the bottom but don’t sign until you are in front of an event volunteer.

The last sheet on the bottom of your stack will be the ACA Insurance Waiver. There are six boxes at the top and you need to check one off. If you are an ACA member with a valid number, check the box on the far left in the upper row. If you are not an ACA member, check the box at the far right of the second row. Do not check any of the other boxes. If you are not a member, bring five dollars per person with you (correct change is appreciated). Fill out all the rest including the ACA number if any. Fill in everything except the signature. Don’t sign it until you are in front of an event volunteer.

Exception for minor waiver: fill it out and sign it in front of a notary. Note that a minor needs the other two forms too.

When the three forms are filled out for each person in your boat, staple them together in the proper order with a group of three for each person:

1. Top – Float Plan
2. Middle – WaterTribe Waiver
3. Bottom – ACA Waiver

Bring them with you on inspection day along with $5 per person.
